Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

As improvements in neuroscience have enabled a better understanding of disorders of consciousness as well as methods to treat them, a hurdle that has become all too prevalent is the denial of coverage for treatment and rehabilitation services. In 2011, a settlement emerged from a Vermont District Court case, Jimmo v. Sebelius, which was brought to stop the use of an “improvement standard” that required tangible progress over an identifiable period of time for Medicare coverage of services. While the use (...) of this standard can have deleterious effects on those with many chronic conditions, it is especially burdensome for those in the minimally conscious state, where improvements are unpredictable and often not manifested through repeatable overt behaviors. Though the focus of this paper is on the challenges of brain injury and the minimally conscious state, which an estimated 100,000 to 200,000 individuals suffer from in the United States, the post-Jimmo arguments presented can and should have a broad impact as envisioned by the plaintiffs who brought the case on behalf of multiple advocacy groups representing patients with a range of chronic care conditions. Comprehending the origin of marine invertebrate larvae remains a key domain of research for evolutionary biologists, including the repeated origin of direct developmental modes in echinoids. In order to address the latter question, we surveyed existing evidence on relationships of homology between the ectoderm territories of two closely related sea urchin species in the genus Heliocidaris that differ in their developmental mode. Additionally, we explored a recently articulated idea about homology called ‘organizational homology’ (Muller 2003. In: Muller GB, Newman SA, (...) editors. Origination of organismal form: beyond the gene in developmental and evolutionary biology. Cambridge, MA: A Bradford Book, The MIT Press. p 51–69. ) in the context of this specific empirical case study. Applying the perspective of organizational homology to our experimental system of congeneric echinoids has led us to a new hypothesis concerning the ectoderm evolution in these species. The extravestibular ectoderm of the direct developer Heliocidaris erythrogramma is a novel developmental territory that arose as a fusion of the oral and aboral ectoderm territories found in indirect developing echinoids such as Heliocidaris tuberculata. This hypothesis instantiates a theoretical principle concerning the origin of developmental modules, ‘integration’, which has been neglected because the opposite theoretical principle, ‘parcellation’, is more readily observable in events such as gene duplication and divergence (Wagner 1996. Even today, a century after the publication of the “Origin of Species”, current zoological literature often reveals an insufficient grasp of the implications of the now generally accepted view that it is natural selection that confers direction on the evolutionary process.This is, in part, due to a reaction against oversimplified teleology and against Lamarckism. In rejecting Lamarck's thesis that the activities of an animal directly affect its hereditary characters it is frequently assumed that this implies that such activities are irrelevant (...) to the study of evolution. This is a non-sequitur, for activities may affect evolution not directly, through heredity, but indirectly, by influencing the direction of the selective forces impinging on the organism. Reasons are given for concluding that changes in habits and behaviour frequently precede structural change and, in fact, determine the direction of the latter.As an example of a concept which deserves a more evolutionary treatment than it commonly receives, the subject of neoteny is considered. It appears that failure to think in terms of the selective forces involved has frequently led to error. An analysis in functional terms is made of the theory that the chordates represent secondarily glorified sea-squirts and it is concluded that this idea is unlikely to be correct. The characteristics of man that are commonly considered to be neotenous are also discussed. It is concluded that a facile and unanalytical application of the label “neoteny” to many of the evolutionary changes involved has tended to obscure rather than to clarify their significance: the neotenous features of man are not those that have been of primary importance in human evolution, but are secondary to the acquisition of new characters which are not to be found in either the adult or the young stages of any other primate.Aujourd'hui, un siècle après l'apparition de l'Originedes Espèces, la conviction est presque générale que la sélection naturelle dirige le procès de l'évolution. Il en résulte que, en désignant légèrement et sans analyse plusieurs changements évolutionnaires comme néoténie, on a plutôt obsurci qu'éclairci leur signification. Es werden Argumente gegeben, welche die Konklusion rechtfertigten, dass Änderungen in Gewohnheit und Verhalten oft Strukturänderungen vorhergehen und tatsächlich die Richtung dieser letztgenannten bestimmen. Die Neotenie wirdt als ein Beispiel eines Begriff betrachtet, welcher eine mehr evolutionäre Behandlung verdient als bisher geschehen ist. Es ergibt sich dass man viele Irrtümer hat gemacht, indem man den betreffenden selektiven Kräften keine Rechnung getragen hat. Eine Analyse der Theorie, welche die Chordaten als neotenische Ascidien betrachtet, wird von einem funktionellen Gesichtspunkt aus durchgeführt. Daraus scheint der Schluss berechtigt, dass diese Theorie falsch ist. Auch werden die menschlichen Kennzeichen, welche gewöhnlich als neotenische betrachtet werden, besprochen. Die Konklusion lautet, dass, wenn man leichtfertig und ohne genügende Analyse den Zettel „Neotenie“ für mancherlei evolutive Änderungen anwendet, man die richtige Bedeutung dieser Erscheinung vielmehr verdunkelt als aufklärt: nicht die neotenische Kennzeichen des Menschen sind am wichtigsten in seiner Evolution; in Gegenteil sie erscheinen nur sekundär, nachdem die neuen Kennzeichen, welche sich weder in den Erwachsenen noch in den jüngeren Stadien von den anderen Primaten finden lassen, erschienen sind. The historical origin and the experimental basis of the concept of physical determinism indicate that this basis was removed with the acceptance of the kinetic theory of matter, while its difficulties are increased by the admission that human nature, in its entirety, is a product of natural causation. An indeterministic view of causation has the advantages (a) of unifying the concept of natural law in different spheres of human experience and (b) of a greater generality, which precludes the acceptance of (...) the special case of completely deterministic causation, so long as this is an unproved assumption. It is not inconsistent with the orderliness of the world, or with the fruitful pursuit of natural knowledge. It enriches rather than weakens the concept of of causation. It possesses definite advantages with respect to the one-sidedness of human memory, and to the phenomena of aiming and striving observable in man and other animals. Among biological theories it appears to be most completely in harmony with the theory of natural selection, which in its statistical nature resembles the second law of thermo-dynamics. In an indeterministic world natural causation has a creative element, and science is interested in locating the original causes of effects of special interest, and not merely in pushing a chain of causation backwards ad infinitum. These contrasting tendencies are illustrated by a critique of the mutation theory, and by an attempt more closely to define the sense in which indeterministic causation should be thought of as creative. Philosophers and historians of the French Revolution have seen Rousseau's influence as the decisive link between the doctrines of the Enlightenment and the practice of its revolutionary disciples. Professor Leigh here addresses the bibliographical foundations of that question, without which all attempts to settle it in the past have lacked authority. Introducing the most advanced techniques to identify variant and pirate editions of Rousseau's writings, he establishes that there were at least 28 separate imprints and an additional 12 reprints of (...) the Contrat Social in collective editions between 1762 and 1783. Professor Leigh shows also that Rousseau's life and thought excited a fascination and interest in the last years of the Ancien Regime which was nursed by the publishers of his Oeuvres, who sought to satisfy an apparently ceaseless demand by extending their editions, while at the same time attempting to ward off both their creditors and their imitators. I shall describe yet another problem about fiction, similar in some respects to the ‘paradoxes of fiction’ on which so much ink has been spilt over the last quarter of a century. Since fictions are ‘made up’, what considerations stop us from making up our own endings to a fiction which is incomplete or whose ending we have lost or missed or whose ending is unpalatable?

When women confront women through pro-life rhetoric." -/- In this chapter, I articulate a specific problem in the way the rhetoric and ideology of pro-life politics operates as a form of confrontation between women. This is a dilemma that emerges when women engage in the appearance of concern and solicitude while passively coercing other women as they may be ambivalent and vulnerable in forcing anti-abortion outcomes. This in a reinvestment in the (...) problem I raise in my prior work, The Pregnancy ≠ Childbearing Project: A Phenomenology of Miscarriage (2017), a dilemma that I think is essential to grapple with if not also to urgently reconcile: where is the solidarity among women as they are pregnant, independent of the outcomes or expectations of childbearing? Alongside of the testimony of my own personal experience, I analyze the substance of the confrontation narrated in Maisie Crow’s documentary Jackson (2016), juxtaposing it with the fictional hypothesis of Margaret Atwood’s The Handmaid’s Tale (1998) to outline the character of harm in the confrontations between women through pro-life discourse. On the other hand he has been called the first and greatest Stoic mystic who under Oriental influence spurned the body as vile and earthly. Reinhardt has of late years resolutely maintained that the importance of Posidonius in the history (...) of thought lies in his having originated a completely new Vitalism, and that his conception of the world is one in which ‘Subjekt und Objekt, Geist und Wissen, Mensch und Gott, νος und ζω durch eine im Bewusstsein neu erwachte Kraft sich einen und durchdringen: durch die “Sympathie.”’ Among other German scholars Geffcken holds that Plotinus borrowed much from Posidonius, and Jaeger roundly declares that if Posidonius had but found a place for the Platonic Ideas, there would have been nothing left for Plotinus to find. With the publication of this volume from the prolific pen of one of Germany's outstanding younger philosophers, the German-speaking scholarly world has a more extensive survey of key issues in the philosophy of science than the English-speaking world. The book is the first of a comprehensive work whose title is "Problems and Results in the Philosophy of Science and Analytic Philosophy." While the title of the book under consideration shows that it is primarily concerned with scientific explanation and justification, Stegmüller (...) not merely undertakes to acquaint his readers with the most important Anglo-American developments concerning these topics, but also independently develops his own ideas with respect to them and an important, if brief, feature of the volume is an appendix in which he sets forth over thirty "problems that are either unsolved or the solution of which is controversial." Without going into substantival detail, perhaps the quickest way to indicate Stegmüller's main concerns and general orientation is to note the extent of the page references in the index of names where, in descending order, one finds the names of Carl G. Hempel, to whom the volume is dedicated, R. Carnap, N. Goodman, N. Rescher, W. V. Quine, P. Oppenheim, E. Nagel, W. Dray and K. Popper. Curiously, Stegmüller indicates that, aside from Hempel, the man who has exercised the greatest influence on him has been Sir Karl Popper, but then he limits his consideration of him mainly to Popper's early exposition of explanation by reference to the breaking of a thread, while ignoring what he has to say about such topics as falsification, induction, and the problem of demarcation. Under these circumstances it is not surprising that there are no references to P. Duhem, J. Agassi, and P. Feyerabend, but there are also none to G. Bergmann, M. Bunge, P. Frank, N. R. Hanson, A. Koyré, V. Kraft, G. Radnitzky, and J. J. C. Smart, to mention just a few.--R. F. M. (shrink)
